Case Management
Case Managers coordinate patient care so that it is safe, timely, efficient, and client-centered. Our team follows each patient from admission to discharge, ensuring that all the details are accounted for each step of the way. Skilled Nursing
Registered Nurses and Licensed Vocational nurses coordinate medical care for patients between all necessary services and therapies in partnership with the physician. Nurses can help with hygiene tasks, medication management, and wound care in addition to monitoring the overall safety of each patient.
Speech Therapy
Speech Therapists (also sometimes called Speech Pathologists) work with patients to help them recover the ability to communicate and/or swallow following procedures or illness. Therapists can focus on speech articulation, voice quality, aphasia, cognition, language, food choices, and swallowing tecniques.
Social Work
Our Social Workers focus on taking advantage of the appropriate and available community resources so that patients have the best chance of staying safely in their own homes. Social workers may identify needed medical equipment or caregiving roles for our patients. Sometimes, Social Workers help coordinate alternative living situations such as nursing homes or assisted living should home not be safe.
